Quality
In November 2019, the most recent Medicare survey took place by the Ohio Department of Health. This Home Care program was found to be deficiency free and compliant with all applicable Medicare program requirements. Employees are full-time and salaried with an average of 25 years home care experience. The staff provides service as a multi-disciplinary team of caregivers to coordinate and manage patient care.
Availability
Our focus for adults is to provide quality skilled home care services to those who are uninsured or under-insured, Medicaid pending, or have limited health coverage/high deductibles.

- Adult Program - 5 days a week, 7:30 AM to 4:00PM.

Anyone can request a home care referral by calling our office at (937) 225-4500. A registered nurse will coordinate home care with the patient’s physician.
Yes, a physician’s order is required to provide home care.
Nursing services including wound care, disease management (diabetes, COPD, heart failure) and medication teaching. Speech therapy and nutritional consultations are also provided.
There is no charge for services.
